What?

Aged care is home away from home. A support to families and their loved ones to provide ongoing care, when this can no longer be achieved at home due to varied circumstances.

Your ongoing support and involvement in your loved one’s lives will be supported to provide holistic care and quality of life. We support and encourage partnership in care program for ongoing involvement and inclusion of care and companionship needs, as we foster trust and good relations.

We offer holistic care and provide 24-hour Registered Nurse support and care.

Types of care

  • Respite care – temporary stay

  • Memory support – Dementia support

  • Palliative care – end of life care

 

We have the ongoing support of a visiting Medical Practitioners, Geriatricians, Dermatologist, Pathology, Podiatry, Speech Pathology and Dietitian and inhouse Physiotherapy for ongoing support of individual assessment and reviews.

Who?

Eligibility to receive government-funded aged care services is determined by the Commonwealth government. You may be eligible to receive services if you;

  • are 65 years of age or older (50 years or older if you identify as an Aboriginal or Torres Strait Islander person)

  • need help to do the things you used to do.

How?

To gain access to our facility you are required to have an assessment/ support plan.

This can be obtained from a Social Worker who will assess the person requiring aged care placement. This assessment includes information about your assessed needs and approval for you to move into an aged care facility.

You are also advised to complete an Income/ means test assessment, which can be completed through Centrelink. This form is known as the residential Aged Care Calculation of your cost of care (SA457). This will determine your fee structure, depending on your financial situation.

We will in turn support you through the process and provide you with an opportunity to ask question and establish a rapport, during your decision-making process.
